I suspect it's as real as the dubbed sound. I've seen a spate of 'hermaphrodyte' movies that were posted to Usenet ages ago and although good fantasy material all the second dicks are plastic. I suppose it could be diphallus, as Vix says, which Wikipedia http://en.wikipedia.org/wiki/Diphallus describes as extremely rare, but the positioning of the dicks on this guy doesn't look like that in the photo on Wikipedia.
